In Office Periodontal Discount Plan

Membership Plan
- Single Membership: $700
- Renewal: $600
*The first plan payment of $700 does not include the initial deep cleaning. The 15% discount will be applied toward the deep cleaning. Payment for the discount plan and deep cleaning is due the day the deep cleaning is performed.

- Periodic and Comprehensive Exams

**If dental insurance is purchased at any time, this discount plan is automatically terminated. All further treatment will be billed to insurance. There are no refunds for discount plans. Third party payments (Sunbit, CareCredit, etc.) are not allowed in conjunction with the discount plan.
